Mandatory COVID-19 Health Screening

 

For the health and safety of our members, volunteers and staff you will be asked the following questions upon entering our facilities.

 

If you answer “Yes” to any of the following questions you will not be able to enter the facility.

 

Have you experienced any of the following symptoms in the last 14 days?

·      Fever

·      Chills

·      New or worsening cough

·      Shortness of breath

·      Sore throat

·      New muscle aches or headache

Have you or any member of your household been in close contact with anyone experiencing the above noted symptoms in the last 14 days?

Have you or any member of your household traveled outside of Canada in the past 14 days?

Have you or any member of your household been in contact with any individual who has been instructed to self- isolate?

Have you or any member of your household received a positive COVID-19 test or are awaiting test results?
